Thomas Jay Tackett, age 40, of Ashland, died July 17, 2022 at his home. Born October 2, 1981 in Ashland, Kentucky, T. Jay was the son of Samuel L. Tackett and Johnda Smith Tackett. He died after a valiant battle with severe mental illness. He graduated from Paul G. Blazer high school and attended ACTC. T. Jay loved cooking, dogs, and putting his horrible poetry on Facebook. He was preceded in death by his father. He is survived by his mother, his brother, Bart Tackett, his grandmother Mary Hunter Smith, and many aunts, uncles, and cousins. There will be no service. In lieu of flowers memorial donations may be made to: CARES P.O. box 1503, Ashland, Ky 41105. Lazear Funeral Home is both honored and privileged to have been entrusted with the care of Mr. Tackett.
